Adult Services
The Adult Service Program has many different services that person can choose from. Our programs are driven to ensure support for persons with physical, cognitive and emotional disabilities. These programs are geared to help support individuals with disabilities to live happily, enrichingly and independently at the best of their ability. The programs are set up to occur periodically and/or up to 24 hours daily.
Residential Hab (ResHab).
Our ResHab program are design to support individuals with acquisition, retention and/or improvement in skills for better living in the community. ResHab provides supports to make sure individuals live at the highest level of independence by providing money management, shopping self-care, medical support assistance and home care. Services are provided from 6 hours to 24 hours per day.
DSP
Our Direct Support Professionals (DSPs) provide services to individuals based on their specific goals and needs. We provide supports within a person’s own home or apartment. Some of the assistance our staffs provide include but no limited to assisting with personal care, medical maintenance, cooking meals, as well as home care responsibilities, and other needs of individuals on a case-by-case basis.
Case Management
We ensure that a person receiving services is assigned a case manager. We maintain that every person is unique. Case managers make sure to provide the supports needed and required for each person to succeed on a case-by-case situation. Responsibilities of case managers include assisting with advocating for the rights of the individual, counseling, acquisition and maintenance of benefits and money management as well as training and communicating with Direct Support Professionals to ensure the person is receiving the best services.
